Form: Informed Consent to Therapy

What To Expect: In your first session we will review the informed consent and discuss the concerns that brought you in. Once intake and assessment are completed, all future sessions will be focused on identifying and reaching your therapeutic goals. 

Payment: Clients are responsible for payment at the time of service. Cash, check, debit and credit cards are accepted. 

Insurance: I am an out-of-network provider, and do not accept payment from or bill any insurance providers. Please contact your insurance provider to learn if they offer an out of network reimbursement benefit. I am able to provide a monthly super bill, which includes your personal contact information, a clinical diagnosis, dates of service and the amount you paid per session. 

Cancellation Policy: If you are unable to keep an appointment, please call to reschedule 24 hours in advance. Outside of an emergency, the full session fee will be charged to clients who late cancel or do not attend their appointment.

Reduced Rate: A reduced rate may be offered, depending upon availability. Please let me know if this is a concern for you.
